2024 HOLIDAY GIVEAWAY OFFICIAL RULES

1. NO PURCHASE NECESSARY TO ENTER OR WIN:The Solutionreach 2024 Holiday Giveaway (“Sweepstakes”) is open to legal residents of the United States, age 18 years or older, except employees, officers, directors, members, managers, and agents and their immediate families (spouse, parent, child, sibling and their respective spouses) and household members (whether or not related) of Solutionreach, Inc. (“Solutionreach”), and its parents, affiliates, officers, directors, managers, members, trustees, participating vendors, distributors, judges, advertising and promotion agencies and affiliated entities (collectively, with Solutionreach, “Sweepstakes Entities”). Due to legal restrictions in Canada, residents of Canada are not eligible for the Sweepstakes. Void wherever prohibited or restricted by law. Sweepstakes is governed by U.S. law and subject to all applicable federal, state and local laws and regulations. No purchase is necessary to enter or win.

   By entering the Sweepstakes, entrants agree to accept and be bound by all terms of these official rules (“Official Rules”) and understand that they and/or their Submissions (as defined below) may appear in any advertisement associated with the Sweepstakes or Solutionreach. 

2. HOW TO ENTER: Beginning at 12:00:00 am Mountain Time (“MT”) on December 9, 2024 and ending at 5:00:00 pm MT on December 20, 2024 (the “Promotion Period”), entrants may enter the Sweepstakes: (A) by signing up for a Solutionreach software subscription within the Promotion Period (new customers only); or (B) by printing your full name, complete address, phone number (including area code), and healthcare practice name on a 3×5 card and mailing it in an envelope with proper postage affixed to: Solutionreach, Inc. 2600 N Ashton Blvd., Suite 200A, Lehi, UT 84043. Limit one (1) entry per eligible entrant per healthcare practice. Solutionreach reserves the right in its sole discretion to disqualify any individual who tampers with the entry process. Entries may be subject to verification prior to prize drawing. Entries become the property of Solutionreach and will not be returned. Solutionreach is not responsible for lost, late, misdirected, damaged or incomplete entries, or for transactions that are processed late or incorrectly computed or are lost, all of which are void, because of computer error or malfunctions.

3. RANDOM DRAWING: On or about December 23, 2024, Solutionreach will select potential winners in a random drawing from among all eligible entries received. Entries must be received during the Promotion Period to be eligible to win. The random drawing will be conducted by Solutionreach whose decisions are final on all matters relating to this Sweepstakes. Potential winners who meet eligibility requirements and comply with all Official Rules will be named a prize winner (“Winner”).

4. ODDS: Odds of winning a prize will depend on the total number of eligible entries received. PRIZE: Five (5) Winners will win a $1000USD Gift Card. TOTAL VALUE OF ALL PRIZES: Approximately $5000 USD. No substitution or transfer of prizes or cash redemptions permitted by the Winner. Solutionreach reserves the right to substitute for any reason whatsoever a prize (or portion thereof) of comparable or greater value. Prizes are awarded “as is” with no warranty or guarantee, either express or implied by Solutionreach. The Winner is responsible for the reporting and payment of all taxes (if any) as well as any other costs and expenses associated with acceptance and use of a prize not specified herein as being awarded. Any gift cards are subject to retailer regulations and restrictions. All prize details are at Solutionreach’s sole discretion.

5. GENERAL RULES AND REGULATIONS: By entering this Sweepstakes and/or accepting the prizes, participants grant Solutionreach and its designees the right, unless prohibited by law, to use their names, cities and states of residences, voices, performances, pictures and likenesses (including recorded media coverage), without compensation, notification or approval, for the purpose of advertising, promotional, and other purposes, in any manner or medium, now or hereafter known, throughout the universe in perpetuity.

   The potential Winners may be required to execute an affidavit of eligibility, liability release, tax acknowledgment form and, except where prohibited by law, publicity release and return within five (5) business days of date of issuance of notification. Noncompliance within this time period or failure of a potential Winner to respond to a notification attempt within five (5) days of such attempt may result in disqualification and, at Solutionreach’s discretion, a new Winner will be drawn at random. If: (i) any prize notification is returned as undeliverable, (ii) a potential Winner declines his or her prize; or (iii) a potential Winner fails to comply with any of the Official Rules as outlined herein, he/she will be disqualified and, at Solutionreach’s discretion, a new Winner will be drawn at random.

   THE SWEEPSTAKES ENTITIES, AND THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES AND AGENTS (COLLECTIVELY, THE “RELEASED PARTIES”) EXPRESSLY DISCLAIM ANY RESPONSIBILITY AND PARTICIPANTS AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASED PARTIES FROM AND AGAINST ANY AND ALL CLAIMS, ACTIONS, DEMANDS AND/OR LIABILITY FOR INJURY, DAMAGE OR LOSS WHATSOEVER RELATING TO OR ARISING IN CONNECTION WITH PARTICIPATION IN THIS SWEEPSTAKES (REGARDLESS OF THE CAUSE OF SUCH INJURY, DAMAGE OR LOSS), SOLUTIONREACH’S USE OF ANY MATERIALS SUBMITTED BY PARTICIPANT, AND/OR THE DELIVERY AND/OR SUBSEQUENT USE OR MISUSE OF ANY OF THE PRIZES AWARDED.
